LOCATION: North & Rotating Health Centers
DIVISION/DEPARTMENT: Clinical Services
SALARY INFORMATION:  $17.00 Hourly / Pay Grade TPW / Job Code T1001 

Job Responsibilities

Under supervision, conducts research involving data collection and analysis, develops and implements policy, and administers social services programs.  Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties.   Actively link and guide clients with information and strategies on how to access HIV/AIDS treatment and support services. Other duties in relation to this overarching objective are:

  • Assist clinic staff in linking clients to resources identified related to health insurance enrollment, financial assistance, medication, home care, transportation and other social services.
  • Identify and support clients and other health center staff in addressing client barriers to care.
  • Follow-up with clients who miss appointments for HIV/AIDS medical and support services.
  • Provide basic HIV/AIDS education and education resources to newly diagnosed clients.
  • Offer emotional support and make referrals to support groups and classes.

Minimum Qualifications

Bachelor's degree in a related field from an accredited college or
university 
OR
Four years of related experience.
