Position Overview:
Under the supervision of the Maternal and Child Health Program Manager, the Social Work Supervisor is responsible for providing support and administrative supervision of community health coordinator(s) and support Black Maternal Health enrolled participants. This program intends to expand resource navigation of social and health benefits for new and expecting families through a systematic approach of care coordination, prenatal care, postpartum care, and infant-early child development. The Social Work Supervisor is accountable for ensuring delivery of exceptional services that are focused on the client needs. Telehealth services may be required in certain circumstances.

Duties & Responsibilities:
  • Supervise and train community health coordinators in the delivery of quality resource navigation services and compliance with agency policy and build and maintain trusted relationships with program participants, partners, stakeholders, and administrators.
  • Communicate participant information and needs with the program manager, assistant program manager, community health coordinator(s), and other staff as needed.
  • Maintain notes and records for each participant interaction and keep a record for each client.
  • Submit reports regularly as requested by the program manager and coordinator.
  • Adheres to cultural competency standards and provides every participant with equitable treatment and service.
  • Create and maintain referrals to community resources and partners and other duties as assigned by the program manager(s) such as special emergency tasks.

Requirements

Education:

  • Master's degree in Social Work, Public Health, Health Education, or related field. 
Experience:
  • Minimum of two (2) years of supervisory experience.
  • Minimum of one (1) year of experience working with the maternal and child population.
Licensure:
  • Valid Driver’s License (Texas upon hire).
Knowledge, Skills and Abilities:
  • Maintain necessary licenser and certifications required to practice in the State of Texas (if applicable).
  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Excellent communication skills (verbal & written).
  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment.
  • Ability to maintain confidentiality when dealing with sensitive information.
  • Ability to work well with public and staff members.

General Information

Position Type and Typical Hours of Work:

  • Full Time | Regular | Grant-funded.
  • Monday through Friday; 8:00 a.m. - 5:00 p.m.
  • Must be willing to flex work hours when required, including weekends.
  • Expected to travel within Harris County jurisdiction.
Work Environment:
  • This job operates in a professional office environment with occasional visits needed to private residential homes, clinics, and other community organizations. Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands:
  • This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
