This is a grant funded position. 

Position Overview:

The Case Manager position is part of the Healthy Start Team and is responsible for providing case management services to the County's most vulnerable residents. The Case Manager will offer ongoing support, referrals, and coordination of services to address participants' needs related to social determinants of health, enhancing stability and overall well-being. Under the supervision of the Healthy Start Program Manager, the Case Manager will complete assessments, develop a plan, evaluate actions/goals, and monitor interventions required to meet cohort participants physical, mental, and emotional health needs or self-sufficiency.


Duties & Responsibilities:

  • Responsible for providing ongoing case management services for participants enrolled;   works with participants to enhance stability and overall well-being by addressing specific needs that may have contributed to the identified vulnerability; assists with internal and external referrals for medical, substance use, mental health, parenting, employment, housing needs, and other needs related to social determinants of health.
  • Responsible for an assigned caseload and  be responsible for the successful case planning for program participants; conduct screening and assessment within designated time-frames and monitor the case for the duration of the program; maintain case records and documentation as appropriate.
  • Provide trauma-responsive case management services, effectively manage crisis situations through assessing, deciding, and acting in accordance with standards and laws in emergencies involving the health and safety of participants being compromised.
  • Complete all pertinent program documents within designated time frames; duties may include intake, enrollment, case management, monitoring, group facilitation,  resource referrals, etc. effectively manage crisis situations through assessing, deciding, and acting in accordance with standards and laws in emergencies involving the health and safety of participants being compromised.
  • It will have an assigned caseload and will monitor the case for the duration of the program and will be responsible for case management of the Healthy Start program participants to ensure an appropriate level of care and develop a participant care plan
  • Conduct site/home visits as needed to complete initial assessments on new participants and provide follow-up to existing participants; Accompany participants to medical, housing, and other needed appointments, and any other in-field support requested; communicate with members of the community, colleagues, and clients effectively and efficiently.
  • Facilitates case coordination and information sharing amongst safety net departments, community-based organizations, and any additional stakeholders; Provide continuous feedback to the Care Coordination Team during case conference; maintains case records and documentation as appropriate.
  • Support participant engagement in services, including following-up with external service providers including but not limited to physical and mental health, substance use treatment, parenting support, and educational programming; helps participants make informed decisions by acting as their advocate regarding their clinical, emotional, nutrition, and life skills status; intervenes with treatment options when necessary; support teammates when necessary.

Requirements

Education

  • Bachelor's degree from an accredited college or university majoring in Public Health, Public Relations, Social Work, or a related field.

Experience: 

  • At least one (1) year of experience working as Case Manager or in Case Management.

 

Licensure:

  • Valid Texas Driver's License upon hired.

Preferences

Education: 

  • Master's degree in Business Administration, Public Administration, Public Service & Administration, Health Administration, or related field.

 

Experience: 

  • One (1) year of supervisory/management experience.
  • One (1) year of using a case management software.

General Information

Position Type, and Typical Hours of Work:

  • Full Time | Regular | Grant-funded
  • Typically, Monday through Friday, from 8:00 a.m. - 5:00 p.m.; 40 hrs per week (Including evenings and weekends).
  • Ability to work outside regular business hours (evenings and weekends) is required.
  • May be required to travel which may include transporting clients, participating in community outreach programs, and attending meetings and training opportunities.

 

Work Environment:   

  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. Staff will conduct visits to participants homes using the agency vehicles. This job will require about attending periodic events in the community

 

Physical Demands:

  • Frequently required to sit or stand, use manual dexterity, speak, listen, hear, and write. Works in an office or indoor environment with little or occasional light physical effort and moderate noise. May occasionally lift and move up to 50 pounds.
