Community Outreach Worker
The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.
This position will serve as a health advocate for the Gaston County Haitian community and a liaison between this community and Gaston County Public Health by:
- Building trust with the Haitian community and connecting community members to health services through meeting with community members to provide education, holding educational sessions, attending/participating in events where this population may be, helping community members make appointments and navigate systems as needed; and making recommendations to improve system access for this community.
- Ensuring Public Health communication is culturally and linguistically appropriate for the Haitian Community by: managing social media content, helping to translate documents and signage, participating on the Communications Team and engaging in other communication efforts.
- Work with DHHS staff and community partners to: develop appropriate strategies to address the needs of the local Haitian population; review internal public health programs, policies and practices to increase access and appropriateness for the Haitian population; develop culturally relevant materials and outreach strategies as needed.
- Implement strategies aimed at achieving Health Equity, including but not limited to: attending trainings to better understand inequities, identifying and addressing barriers to health, serving on the Health Equity Team or its subcommittees, engaging in internal or external efforts to eliminate health disparities.
- Participate in required supervisory meetings, staff meetings, and other related activities conducted by the Department of Health & Human Services and its Public Health Division.
- As needed, assist in preparedness and emergency response activities in situations that threaten the public's health (i.e. disease outbreaks, natural disasters, etc).
- Carryout Performance Management (PM) roles and responsibilities, including identifying and communicating performance improvement opportunities and adjusting performance and practice to achieve established PM goals.
- Other duties as assigned
Graduation with a high school diploma or GED equivalency with at least six (6) months of public health, social worker, or related experience is required. Must be fluent in both English and Haitian Creole with strong interpretation and written translation skills.
A Bachelor's degree in Public Health, Social Work, Human Services, Counseling, or a related health or education field preferred.
Experience may be accepted in lieu of a degree.
