This vacancy is located within the Pharmacy at Eastern Louisiana Mental Health System in Jackson, LA (East Feliciana Parish). ELMHS is the largest 24-hour, multi-site, state psychiatric complex comprised of residential, outpatient and inpatient services.

This position will be filled as Probation, Promotion, Demotion, or Lateral Transfer.  This posting may be used to fill future vacancies

Requirements

MINIMUM QUALIFICATIONS:

Possession of a current Louisiana license to practice pharmacy plus one of the following:

Two years of experience as a licensed pharmacist; OR

Possession of a master's degree in pharmacy plus one year of experience as a licensed pharmacist; OR

Possession of a Ph.D. in pharmacy.

Qualifications

Function of Work:
To perform professional level pharmacy tasks.

Level of Work:
Advanced.

Supervision Received:
General from a Pharmacist 4 or other high-level health care personnel.

Supervision Exercised:
May exercise functional supervision over pharmacist interns and lower-level pharmacy support personnel.

Location of Work:
May be used by all state agencies.

Job Distinctions:
Differs from the Pharmacist 2 by the presence of advanced duties and/or lead-worker responsibilities.
 
Differs from Pharmacist 4 by the absence of supervisory duties.

Differs from Pharmacist 5-A by the absence of serving as a pharmacist-in-charge.Dispense drugs to wards, clinics, and clients using proper pharmacy procedure.
 
Assure the monitoring and evaluation, with medical staff input, of the quality and appropriateness of patient services provided by the pharmaceutical department or service.
 
Prepare IV piggybacks, large volume parental, chemotherapy solutions, and parental nutrition hyperalimentation solutions.
 
Participate in patient care rounds to provide drug information regarding therapeutics and medical problems or questions.
 
Assist in instructing and training staff Pharmacists, residents, and medical and other health care students in therapeutics and clinical pharmacy functions.
 
Participate in aspects of hospital quality assurance program relating to drug utilization and effectiveness.
 
Provide drug monitoring services which may include monitoring pharmacokinetic drug levels.
 
Instruct patient or appropriate nursing staff personnel who advise patient, verbally or in writing, on importance, correct use, and possible side effects of medication to be taken following discharge, in accordance with physician's instructions or written medical staff policy.

Reviews patient's drug regimen for potential interaction, interferences, or incompatibilities prior to dispensing drugs to patient, resolves irregularities with prescribing physician, and notifies nursing service and hospital administration when appropriate.
 
Collect information in the pharmaceutical department/service, or through hospital quality assurance program, concerning aspects of pharmaceutical services.
 
Assess data to identify problems in patient care and opportunities to improve care.
 
Determine usage patterns for drugs according to clinical department or individual prescribers and assists in setting drug use criteria.
 
Communicate new product information to nursing service and other hospital personnel, as required.
 
Participate in meetings of the pharmacy and therapeutics committee and implements decisions of that committee.
