Preference will be given to candidates for this position (My Choice LA Peer Support Specialist Program Monitor) who meet the following criteria:
- Experience working with individuals with behavioral health conditions and complex medical needs.
- Experience with one or more of the following: transition planning from nursing facilities; support coordination/case management; social services coordination; Medicaid or program office function on the state or regional level; knowledge of the Medicaid and MCO system of care.
- Excellent organizational and communication skills (written and oral).
- Demonstrated computer proficiency.
- Ability to maintain favorable public relations, and provide community leadership.
- Demonstrated ability to work effectively with culturally diverse staff and populations.

Requirements
Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

This is a highly responsible position within OBH’s Division of Health Plan Management (HPM). This position functions under the direct supervision of the Program Manager 2 over Community Integration/Transition Coordination, which is located within the Community Integration/Best Practices section of HPM. The incumbent employee in this position acts as principle staff to the organization and implementation of Peer Support services, working in conjunction with My Choice Louisiana Transition Coordinators throughout the state. In this role, the Peer Support Specialist will oversee a cadre of Peer Support Specialists, employed as WAEs or contractors, supporting individuals identified through the My Choice Louisiana program as they prepare to transition into the community for up to 365 days post transition. This includes collaboration with members and their supporters, transition coordinators employed through OBH and the Office of Aging and Adult Services (OAAS), Managed Care Organizations (MCOs), and other community partners. ‘The incumbent in this position will support transition process in compliance with established guidelines. As needed, this responsibility extends to directly advocating for and assisting individuals, organizations, and providers in the process of transition (direct service). Other responsibilities can include providing individualized support, disseminating information, providing technical assistance, and facilitating resolution of individual transition issues. The incumbent will participate in policy development and planning at the state office level in relation to the use of peer support with this program in order to complete longitudinal planning to maintain or enhance community capacity to support attainment of transition targets. This will include combining state and local initiatives and resources to support the transition of individuals from nursing facilities. This may also include participation in policy development and planning that involves multiple administrative units within the Louisiana Department of Health and across multiple state agencies.
ESSENTIAL FUNCTIONS:
Responsibilities include supporting nursing facility transitions in compliance with developed standards for operation. This may include, but is not limited to:
- Provide in reach within Nursing Homes to its residents, inquiring about their interest in transitioning into the community.
- Conducting non-clinical screenings intended to identify individuals interested in transitioning into the community.
- Providing ongoing communication with the residents and ensuring a person-centered plan for discharge is identified, also ensuring wellness and crisis plans are developed.
- Maintaining documentation as well as monitoring/updating spreadsheets for accuracy and documentation of progress in transition.
- Participation in scheduled conference calls with MCOs, providers, OAAS and OBH staff, as well as any other relevant entity to assist in the successful transition into, and maintenance within, the community. This includes participation in state level meetings and committees intended to review mortalities, critical incidents, and/or especially complex situations in which an individual is at risk of re-institutionalization.
- Participate in community outreach activities, community meetings and meetings within nursing facilities, promoting the My Choice Louisiana program. As needed, provide training of Office of State Ombudsman (OSO) staff, and others related to the My Choice Louisiana initiative and the role and purpose of peer support and person centered planning.
- Develop and implement policies and procedures related to the statewide implementation Support Specialists with the My Choice Louisiana Transition Coordination initiative
Provide supervision and oversight to a cadre of Peer Support Specialists employed throughout the state as WAE employees or through contracts. Ensure appropriate training and oversight for these individuals. This includes the assignment of tasks and follow up regarding their successful completion, as well as administrative oversight regarding their positions as it relates to scheduling, time administration, and payroll.
