The Detroit Health Department has been providing important public health services to Detroiters for over 100 years.

The Health Department has three major offices: Population Health, Clinical Services and Outreach, and Administration and Organizational Development. Each office works collaboratively with external partners and the community to create a public health system that supports the health of Detroiters across their lifespan.

This is a Temporary Administrative Special Services (TASS II) position.
This Appointment Term is based on the availability of grant funds.

Requirements

  • Provide comprehensive nursing care and patient support at the Neighborhood Wellness Center.
  • Promoting community health and wellness through personalized and compassionate service.
  • Oversee and provide clinical services at assigned Neighborhood Wellness Centers.
    • Preventive Health Screening
    • Health Education
    • Health Referral Services
  • Performs a variety of basic patient care activities under the direction of a which may in the clinical lead, include, but are not limited to, vaccination of clients (Pediatric and Adults), observation of clients after vaccinations, greeting and registration of clients, escorting clients to vaccination area, performing testing as needed and provided results and completion of vaccination records.
  •  Supervises, including assignment and evaluation of subordinate team members, and provides patient care on a daily basis.
  •  Adheres to the mission, vision, values.
  • Complies with approved policies, procedures, guidelines and standards as established by the Detroit Health Department and the City of Detroit.  
  • Demonstrates an understanding of and a commitment to customer service values of respect, courtesy, ownership, privacy, professionalism and responsiveness.
  • Completes mandatory education and training as defined by the Detroit Health Department and City of Detroit and the specific employer-facility.    
  • Maintains a safe working environment through compliance with established policies and procedures and timely reporting of safety variances. 
  • Demonstrates a commitment to improving the work environment through participation in meetings, timely and effective communication and an embracement of cultural diversity.
 PRINICIPAL / ESSENTIAL DUTIES
 
  • Utilizes various strategies to assess patient, family, employee and environment of care strengths and weaknesses to determine appropriate action plan(s).*
  •  Develops a detailed plan of care, which includes patient / family education, based on initial and on-going assessment findings to optimize patient outcomes.*
  • Assigns, delegates, and supervises other employees and intervenes as appropriate to ensure that the plan of care is carried out and that established outcomes are achieved through availability and appropriate utilization of human, material and financial resources.*
  •  Evaluates the effectiveness of plans and interventions to promote optimum achievement of established goals.*
  • Collaborates formally and informally with other members of the healthcare team, including facility and DHD representatives, to identify opportunities for improvement and develop strategies for enhancing clinical practice.*
  • Establishes relationships with professionals in other healthcare and community organizations to develop strong partnerships that support improved patient outcomes and the health of the community served.*  
  • Provides supervision, including guidance, counseling, teaching and mentoring, to targeted peers and subordinates to enhance learning experiences and improve the professional practice environment within the designated work setting.

Qualifications

Registered Nurse (RN)
  • Licensed Registered Nurse
  • Must be BLS Certified
  • 1 year of experience
Licensed Practical Nurse (LPN)
  • Valid LPN licensure State of Michigan
  • Must be BLS Certified
Equivalency
  • Equivalent combinations of education and experience that provide the required knowledge, skills, and abilities will be evaluated on an individual basis. 
KNOWLEDGE, SKILLS & ABILITY:

REGISTERED NURSE: 
  • Advanced patient assessment and critical thinking skills
  • Care planning and implementation
  • Strong communication and leadership abilities
  • Patient education and health promotion expertise
  • Collaboration with interdisciplinary teams
LICENSED PRACTICAL NURSE: 
  • Works within a variety of health and community settings to assess health needs 
  • Vital signs and point of care testing for screening
  • Health Education and assessments, conduct comprehensive assessments of individual and community health needs and monitor and record vital signs such as blood pressure, heart rate, and temperature.
  • Critical thinking skills and leadership skills
