All Coastal Health District, County Health Department employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.
Requirements
Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services- Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others and appropriate community agencies
- Implements and organizes the delivery of specific social services within the community
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress
- Maintains program statistics for purposes of evaluation and research
- Monitors patient's/client's/consumer's progress towards treatment goals
- Prepares intake reports and case history records
- Provides short-term case management and referral services to clients with emergency situations
- Maintains records and supplies
- Participate in Community Awareness activities
Qualifications
High School diploma/GED and two (2) years of job- related experience in a social services-related position; or One (1) year of experience at the lower level Social Svcs Tech 1 (SST010) or equivalent position.
Preferred Qualifications:
2 or more years previous experience working in a community outreach, social services, or public health position is preferred. Associates Degree in a Health or Social Services related field and previous experience working with HIV/AIDS/STD population.
